It does sound like the cam chain.

When you replaced it did you clean out the little ball valve at the bottom of the pusher piston and replace the rubber tip?

If not then I'd do this first as it costs little and requires very little dismantling.

The journal bearing head is prone to wear at high mileages, even with decent oil and regular changes 45k seems to be the mark when revved high. (if you did nothing but bimble around town they would prob last forever.

There was a seller on eBay knocking out complete heads with cam and valves for 45 quid but I think they have all gone now.

Be very careful of your squish clearance if you do change the head as your piston will have a slightly different shape to the crown edge (narrower squish band).

You do have a little piston slap too but this can run for a very long time without trouble so long as compression remains OK.
